This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested, and a written Solicitation will not be issued.

Solicitation number 36C24818Q9408 is issued as a Request for Quotation (RFQ). The attached solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-99 dated 6/15/2018. Responses should be sent to shirley.courtney@va.gov.

The North American Industry Classification System (NAICS) code for this acquisition is 238290 and the small business size standard is $15 million. This procurement is 100% set-aside for Service-Disabled Veteran-Owned Small Businesses (SDVOSBs). All offerors who provide goods or services must be registered in the System for Award Management (SAM) located on the web at https://www.sam.gov and registered/verified in the VIP database at https://www.vip.vetbiz.gov/.

The government anticipates awarding a Firm-fixed price contract. An award will be made to lowest price quote, which conforms to the requirements within this solicitation and represents the best price to the government.

Description of Supplies requested: The Veterans Health Administration, Network Contracting Office 08, North Florida/South Georgia, Veterans Health System, Lake City, FL requires Installation of an Operator System for the Anesthesia Room.

Submitting Quote: (See attached Price/Cost Schedule); Interested contractors must submit a copy of any relevant certifications and licenses along with their quote. Quotes must be received July 30, 2018 by 5:00 PM EST. All quotes must be submitted by electronic submission in reference to 36C24818Q9408 to Shirley.Courtney@va.gov. Quotes received after the exact time specified in the solicitation are considered late and may be considered if the action would not unduly delay the acquisition. All questions regarding this solicitation must be submitted to the Contracting Officer in writing (email is preferred).
